Fundamentals of Nursing for NORCET: Vital Signs, Positioning & Infection Control

Concept RNA blog

The Highest-Leverage Subject in the Entire Syllabus

Because Fundamentals of Nursing underlies every clinical subject, its questions appear even inside Medical-Surgical or Pediatric sets, disguised as scenario questions. Getting comfortable with normal vital sign ranges by age group and the correct patient position for common procedures pays off across the whole exam, not just one section: this is arguably the highest-leverage subject in the entire syllabus precisely because its content resurfaces everywhere else.

Vital Signs: Ranges, Not Single Numbers

Vital signs should be learned as ranges, not single numbers, and you should be comfortable identifying when a given reading falls outside normal limits for a specific age group, since paediatric and adult ranges differ meaningfully. Beyond the basic four vital signs, oxygen saturation and pain assessment are increasingly treated as a "fifth vital sign" in NORCET-style questions, so don't neglect them.

Positioning: Learn the Rationale, Not Just the Name

Patient positioning is tested through direct recall (which position is used for a lumbar puncture, or for a patient in shock) and through applied scenarios (why a particular position is contraindicated for a specific patient). Learn positions in pairs with their clinical rationale: for example, semi-Fowler's position for respiratory distress, because it uses gravity to ease diaphragm movement: rather than as an isolated list of names.

Infection Control: Standard vs Transmission-Based Precautions

Infection control is tested at two levels: standard precautions that apply to every patient, and transmission-based precautions (contact, droplet, airborne) that apply to specific conditions. Know which category a common condition falls into: for example, tuberculosis requires airborne precautions: since this pairing of condition-to-precaution-type is one of the most frequently repeated question formats in this subject.

Parameter Normal Range
Pulse 60–100 bpm
Respiration 12–20 breaths/min
Blood Pressure 120/80 mmHg (average)
Temperature 36.5–37.5°C
